QUALIFICATIONS:

1. Formal training in activities for geriatrics. Optional training available.

2. Demonstrate ability to work with long term care residents.

3. Employee must be a patient understanding individual.

4. Employee must be a licensed driver and have a vehicle available throughout the working day.

SKILLS NEEDED:

1. Ability to work with and motivate elderly residents.

2. Lead residents. Communication skills.

3. Must be able to maintain good rapport with residents, staff, families or friends of residents.

4. Must be open to new ideas and be willing to incorporate them into successful activity programs.

5. Shall perform most duties standing or walking, assisting or transporting residents 75% of the work shift.

6. Shall have visual acuity and manual dexterity to document resident care, operate equipment 25% of the shift.

7. Shall be able to adjust work schedule to change and stress situations.

8. Must be willing to work occasionally on weeknights and weekends.

9. Must be able to lift approximately fifty pounds.

JOB SUMMARY:

The activity coordinator helps organize and supervise activity programs in the facility, within the limits determined by the Health Care Team Committee’s review of each individual resident.

JOB DETAIL RESPONSIBILITIES:

1. Demonstrates respect for all residents by observing both the Resident Bill of Rights and Civil Rights Policies.

2. Coordinates an activity program for each individual resident, with the limitations set by the physician and with the input of nurses, aides, social worker, physical therapist or other allied health personnel.

3. May include staff members and the resident’s family in planning and implementing resident’s activities.

4. Schedule and participate in performing activities to develop a sense of usefulness, appropriate to each resident’s abilities and create a monthly activity calendar for the Nursing Home and the Memory Care Unit.

5. Encourages both ambulatory and non-ambulatory residents to participate in the planned activities; to meet each resident’s particular needs.

6. Maintains a record of resident’s attendance and participation in planned activities.

7. Reviews the activity program regularly with the Activity Director.

8. Performs other related duties as assigned by the immediate supervisor and other management as required.

9. Abides by personnel policy of the facility.
